Damon confronts Elena, frustrated by her denial and calling her out for lying to him, to Stefan, and ultimately to herself about what is happening between them. He passionately kisses her, but she pulls away, insisting that she cares about him yet delivering the devastating blow that she loves Stefan and that it will “always be Stefan”.
Earlier in the sequence (or interwoven in edits like this), Damon believes he is pouring his heart out to Katherine (whom he thinks is Elena at first or is confronting directly), offering to let go of the past 145 years of pain to start over. However, Katherine ruthlessly rejects him, telling him the ultimate truth: she never loved him, and it was always Stefan.
Damon’s Heartbreak: Facing the exact same rejection from two different women who look identical, first from his centuries-long obsession (Katherine) and then from the woman he has fallen for in the present (Elena), leaves Damon utterly broken, culminating in his tragic outburst of grief and rage in the boarding house.
